What to report
Any
injury to a player, manager, coach, or
spectator that requires medical attention,
treatment, or first aid, must be reported to
the Viejo Little League Safety Officer. |

When to report:
Within 24 hours of the incident, the
Incident/Injury Tracking Report must be
completed by the team manager and returned to
the Safety Officer. Click

Parents
The Little League Master Accident Policy
provides benefits in excess of benefits
from other insurance programs subject to a $50
deductible per injury. In order to file a
claim, you must complete the Chart is Accident
Notification Form, get the leagues Safety
Officers signature, then forward the form to
Little League Headquarters within 20 days of
the incident.
